The Sioux Lookout Area Primary Care Team (SLAPCT) works in a team environment to provide wrap-around services and seamless primary care to children, youth, adults and families within both the town of Sioux Lookout and the First Nations communities in the Sioux Lookout region; with total catchment area of 32,000+ people. If you are interested in providing primary care services in both the municipality of Sioux Lookout and directly in the First Nations communities, then this is the job for you! The Primary Care Team, a resource to the First Nations communities in the Sioux Lookout region, brings together a diverse group of health care professionals to work together collaboratively to help meet the individual needs of clients. They deliver care to help clients with specific health care goals and needs, ranging from managing chronic disease, to complex health needs, to helping a client deal with a life challenge that is negatively impacting their health. RESPONSIBILITIES: . Collaborates with other health care professionals and community resources to promote comprehensive and coordinated care. . Empower and support clients to improve their ability to perform tasks in their daily living including those who have conditions that are mentally, physically, developmentally or emotionally disabling. . Provision of treatment plans that will enable clients to reach their full function, both physical and psychosocial and work integration potential. . Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders, such as autism spectrum disorder or progressive neurological disorders. . Work with people/family/caregivers who have speech, language, or swallowing disorders, but want to learn how to communicate more effectively with those that do. . Advocate on behalf of clients for service, equipment navigation and funding including provincial services, Non-Insured Health Benefits and Jordan's Principle. . Provide individualized treatment plans to develop, maintain, or augment function using evidence-based treatment modalities. Review and modify programming and treatment as needed. . Provide aural rehabilitation for individuals who are deaf or hard of hearing. . Ability to use a wide range of assessment tools across age groups and create continuous quality improvement activities. . Provision of treatment plans that will enable clients to reach their full functional and reintegration potential in collaboration with all stakeholders (client, treatment team etc.). SKILLS & QUALIFICATIONS . Master's or Bachelor's Degree in Speech Language Pathology . Registration (in good standing) with the College of Speech Language Pathology of Ontario . Computer literate with experience in electronic client file entry . Keen understanding of Indigenous customs and cultures, and experience with children and adults who have multiple needs is an asset . Demonstrated ability to work collaboratively with interdisciplinary health care providers and community partners . Demonstrated clinical reasoning skills and excellent assessment and therapeutic skills. . Strong problem solving, decision-making and time management skills . Excellent oral & written communication skills. . Knowledge in current, evidence-based methods and practices . Experience working in a First Nations community and ability to communicate in one of the First Nations dialects used in the community is an asset .
