The Sioux Lookout Area Primary Care Team (SLAPCT) works in a team environment to provide wrap-around services and seamless primary care to children, youth, adults and families within both the town of Sioux Lookout and the First Nations communities in the Sioux Lookout region; with total catchment area of 32,000+ people. If you are interested in providing primary care services in both the municipality of Sioux Lookout and directly in the First Nations communities, then this is the job for you! The Primary Care Team, a resource to the First Nations communities in the Sioux Lookout region, brings together a diverse group of health care professionals to work together collaboratively to help meet the individual needs of clients. They deliver care to help clients with specific health care goals and needs, ranging from managing chronic disease, to complex health needs, to helping a client deal with a life challenge that is negatively impacting their health. RESPONSIBILITIES: . Collaborates with other health care professionals and community resources to promote comprehensive and coordinated care. . Educate family members/caregivers regarding the impact of disability, injury or disease on the individual and their role in the treatment and recovery process. . Empower and support clients to improve their ability to perform tasks in their daily living and working environments. . Provision of treatment plans that will enable clients to reach their full functional and work integration potential, in collaboration with all stakeholders (Client, Primary Care members, Mental Health workers, etc.). . Conduct developmental screening and assessments for infants and children such as motor and sensory-motor delays. . Conduct clinical observations of gross and fine motor, visual-perceptual-motor, and in hand manipulation. . Prescribes specialized adaptive equipment and teaches proper usage. . Conduct self-care and Instrumental Activities of Daily Living assessments. . Conduct mobility, ergonomic and Assistive Device Program assessments. . Sensitivity to the needs of parents/caregivers and the ability to support and involve them in programming. . Evaluate the home, work or school environment to assess the need for specialized equipment modifications and/or supports. . Provide individualized treatment plans to develop, maintain, or augment function using evidence-based treatment modalities. . Advocate on behalf of clients for service, equipment and funding including services, Non-Insured Health Benefits and Jordan's Principle processes. SKILLS & QUALIFICATIONS . Registration (in good standing) with the College of Occupational Therapists of Ontario . Computer literate with experience in electronic client file entry . Criminal Records Check /Vulnerable Sectors Check . Keen understanding of Indigenous customs and cultures . Demonstrated ability to work collaboratively with interdisciplinary health care providers and community partners . Demonstrated clinical reasoning skills and excellent assessment and therapeutic skills . Strong problem solving, decision-making and time management skills . Excellent oral and written communication and interpersonal skills . Knowledge and proficiency in current, evidence-based methods and practices . Experience working in a First Nations community and ability to communicate in one of the First Nations dialects in the community .
