Nurse Practioner The Sioux Lookout Area Primary Care Team (SLAPCT) works in a team environment to provide wrap-around services and seamless primary care to children, youth, adults and families within both the town of Sioux Lookout and the First Nations communities in the Sioux Lookout region; with total catchment area of 32,000+ people. The Primary Care Team, a resource to the First Nations communities in the Sioux Lookout region, brings together a diverse group of health care professionals to work together collaboratively to help meet the individual needs of clients. They deliver care to help clients with specific health care goals and needs, ranging from managing chronic disease, to complex health needs, to helping a client deal with a life challenge that is negatively impacting their health. Provide comprehensive primary health care to clients including assessment, diagnosis, treatment, referral for specialists or out-of-scope circumstances, and continuity of care and support services. . Order and interpret investigative screenings and tests to aid in diagnosis/ treatment . Provide counselling, education and follow-up with clients to treat and manage illnesses . Determine the presence of existing and potential health problems with a focus on lifestyle, psychological, socio-cultural and environmental risk factors that may influence the client's health status . Complete client medical record within the electronic medical records database in an accurate and timely manner . Collaborate and consult with physicians to address circumstances outside of their scope of practice or exceeding their knowledge and skill. . Will deliver Primary Health Care according to the College of Nurses of Ontario Practice Standard for Registered Nurses in the Extended Class and the SLAPCT Strategic Directions. . Collaborates with other SLAPCT members and health care providers to ensure management, referrals and information sharing to maintain the client's continuity of care. . Monitor the ongoing therapy of clients with chronic, stable illness by providing pharmacological, complementary and counselling interventions . Prescribe therapies to manage the chronic illness . Work with physician services, Approaches to Community Wellbeing, and other areas . Implement strategies to promote health and prevent disease with individuals, families, groups and communities SKILLS & QUALIFICATIONS . Registration (in good standing) with the College of Nurses of Ontario . Nurse Practitioner Certificate from an accredited program. . Current registration in good standing in the Extended Class with the RNAO and the NPAO. . Current membership with the College of Nurses of Ontario and the professional associations, RNAO and NPAO. . 2 years' experience working as an NP . Experience working with children and youth. . Excellent Interpersonal skills including ability to coach and develop others . Previous experience working with First Nations communities considered an asset . Excellent relationship-building skills to initiate and carry out initiatives, and build and maintain relationships with senior management, and internal and external stakeholders . Excellent verbal and written communication skills . Demonstrates the principles of collaborative leadership . Demonstrated ability to be innovative and creative APPLY TODAY!
