Case Manager, RN The Sioux Lookout Area Primary Care Team (SLAPCT) works in a team environment to provide wrap-around services and seamless primary care to children, youth, adults and families within both the town of Sioux Lookout and the First Nations communities in the Sioux Lookout region; with total catchment area of 32,000+ people. The Primary Care Team, a resource to the First Nations communities in the Sioux Lookout region, brings together a diverse group of health care professionals to work together collaboratively to help meet the individual needs of clients. They deliver care to help clients with specific health care goals and needs, ranging from managing chronic disease, to complex health needs, to helping a client deal with a life challenge that is negatively impacting their health. Provision of primary care services by a Registered Nurse as outlined by the Registered Nurses' Association of ON - Practice Standards. The provision of primary care by RN's includes assessments of, the provision of, care for, and the treatment of, health conditions by supportive, preventative therapeutic, palliative and rehabilitative means in order to attain or maintain optimal function . The Case Manager aligns her/his practice with evidence-based strategies that maintain the highest level of continuity for the client . Collaborate effectively with all members of the Primary Care Team and assists as needed . Recommend and adapt teaching to the client's identified health needs, revising treatment protocols as requested . Educate & counsel clients' family members, caregivers to promote function and independence including health promotion and injury prevention . Stays current and aware of opportunities to implement new evidence-based methods of client's assessment and treatment . Participates in self-directed learning to ensure relevance . Handling of sensitive/confidential material strictly in confidence in accordance with office policy/PIPHA . Ability to work independently and to assess/monitor client's level of care . Maintain required professional memberships and professional competence through participation in professional development programs and by self-directed study . Input data/treatment plans within the Electronic Medical Record (EMR) . Provide assistance when requested and any other duties assigned SKILLS & QUALIFICATIONS . Registration in good standing with the College of Nurses of ON . Baccalaureate Degree in Nursing BScN with identifiable community health nursing content . Minimum of 2 years' work experience as a RN . Demonstrated understanding of and competence in, serving culturally diverse populations; knowledgeable of First Nations' people, culture, health priorities & social issues . Knowledgeable & proficient in current evidenced-based methods/practice of primary care delivery, with an emphasis on health promotion and risk reduction . A strong understanding of the social determinants of health and the impact on First Nations is required . Knowledge and sensitivity to First Nations culture as well as the government health policies and regulations as they apply to First Nations . Experience/training in Indigenous cultural competency . Ability to work collaboratively within the Primary Care Team other health care professionals . Proficient in the use of computers including Microsoft Office . Excellent interpersonal, verbal/written communication skills . Ability to function effectively during times of change . Must be willing to travel by various modes . Ability to communicate in one of the First Nations dialects of the Sioux Lookout region is an asset. . Previous experience with an EMR would be an asset .
